1)  Snail Mail by printing and mailing this completed registration form and
mailing it in with your payment.

2)  By Internet by copying this registration form (yellow area) and pasting it in the body of an email, filling in the needed info, and sending it to bbryant33@accn.net, then, make your payment online by clicking on the appropriate buy now buttons below.

 membership fee of $79 (this discounted membership is only for those attending the symposium).

 For those that wanted to take exams on this day but also want to attend this CVN Fellowship Training Program "CVN INSTRUCTOR'S CLINIC,"  we encourage you to attend the CLINIC and allow us to set you up in your own hospital to take your exams once you return home.  This will also give you a few more days to prepare after the symposium. 
I wish to sit for my CVRN Level I and/or Level II Board Certification Exam on October 18th (9:00am-12:00pm)
To purchase your exam, please visit www.cvncertify.org, and in the CVN Board Certification section will have a pay button for the Symposium only.

Exam Fees:
Level I CVRN Board Exams will cost $220 at the symposium.
Level II CVRN Board Exams will cost $150 at the symposium.

(You may sit for both exams on this day, but you must pass level I in order to become certified at level II, even if you pass level II and not level I)
